Early Bird Registration Deadline Approaching for Care Transitions - Key to Population Health

May 17 is the last chance to take advantage of the early bird registration discount for the Care Transitions – Key to Population Health statewide summit. The early bird discount allows members to save $20 off the cost of registration for the event. After May 17, registration increases to $80.

The full-day event is hosted by the MHA Keystone Center and MPRO as part of the Lake Superior Quality Innovation Network and will be held from 8:15 a.m. to 3:45 p.m. June 18 at the VisTaTech Center - Schoolcraft College, Livonia.

Featured speakers will discuss social determinants of health, from addressing real implications to finding solutions with large improvement collaboratives. Breakout sessions will cover care coordination and the emergency room, as well as behavioral health interventions in the acute care setting.

Bruce Spurlock, MD, executive director, California-based Cynosure Health, will serve as the keynote speaker. Spurlock will address the importance of collaborative relationships and how they can fix multifaceted public health issues related to social determinants of health.
