GLPP HIIN Partnership Helps Member Hospitals with Process Improvement

Forming partnerships in healthcare is critical to the transformation that will yield improved outcomes and long-term success.
In 2018, the MHA Keystone Center, on behalf of the Great Lakes Partners for Patients (GLPP), partnered with Northville-based Vlasic & Roth LLC to provide tailored performance improvement plans for metrics that have been identified as high priority to its Hospital Improvement Innovation Network (HIIN) and member hospitals and health systems.
To help hospitals and health systems make positive changes across their organizations, Vlasic & Roth analyzes hospital data and provides methodologies and evidence-based interventions to drive high-impact, measurable and sustainable performance improvement. To date, Vlasic & Roth has helped 14 GLPP HIIN hospitals, conducting 150 virtual meetings and more than 20 site visits.
In one example of its work, Spectrum Health, Grand Rapids, recently held a readmissions-focused summit for more than 300 of its system leaders to discuss ways to prevent unnecessary hospitalizations and reduce readmissions. Additionally, the event focused on integrating the patient and family voice at every level of care to truly provide a culture of continuous learning and improvement.
Vlasic & Roth presented to the group on best practices, improved communication, and how Spectrum Health can achieve its mission of improving the health and well-being of the communities it serves.
Moving forward, Vlasic & Roth will assist Spectrum Health in implementing new care models and an ongoing, multidisciplinary approach to decrease readmissions and support patients as they transition to various settings of care.
