A comprehensive root cause analysis and action (RCA²) training program will be exclusively provided to MHA Keystone Center Patient Safety Organization (PSO) members to assist with improving the RCA process within their organization. The training will be administered by Jim Bagian, MD, PE, and Joe DeRosier, PE, CSP, and is based off the National Patient Safety Foundation’s (NPSF) RCA²: Improving Root Cause Analyses and Actions to Prevent Harm guidelines.
The one-day, interactive training will introduce the RCA² elements and develop the foundation for correctly identifying and classifying events. The training also features education on proper team assembly, RCA² process timing and an applied look into event review methodologies, with attention to development and implementation of effective corrective actions to prevent future harm.
Additionally, Mike Personett president, NextPlane Solutions will discuss the alignment of RCA² processes with PSO data submission.
The event will place June 20 from 8 a.m. to 5 p.m. at Bronson Methodist Hospital, Kalamazoo.
Prior to attending the RCA² training, hospitals will be required to submit the following information to the MHA Keystone Center PSO:
At least two completed RCAs, each no more than one year old, via NextPlan
The organization’s most recent culture survey data
This program has been approved for a total of 10.00 contact hours of Continuing Education credit toward fulfillment of the requirements of ASHRM designations of FASHRM (Fellow) and DFASHRM (Distinguished Fellow) and towards CPHRM renewal. To receive the full 10.00 contact hours, you need to complete all the following criteria:
Attend in-person training (7.00 hours)
Attend the online webinar (1.00 hours)
Complete and submit RCA report using the NextPlane portal (2.00 hours)
Complete the online evaluation survey
